RE: PAPERWORK REQUIREMENTS FOR 2017-2018 SCHOOL YEAR

 

Each year PIAA regulations require that all student-athletes of the Dallastown Area School District receive a physical examination prior to their participation in interscholastic athletics. This physical must be performed and dated no earlier than June 1st and shall be effective, regardless of when performed during a school year, only until May 31st of that same school year. For this upcoming school year, those valid dates are June 1, 2017 – May 31, 2018. Failure to turn in paperwork prior to the start of tryouts for your sport will result in a loss of participation for the upcoming season (until all necessary paperwork is received, and at the discretion of the coach/athletic department).

THIS YEAR DALLASTOWN IS GOING ONLINE for your convenience and to help eliminate the paperwork pile-up!!! Before all physicals can be completed, it is required that each student has a SportsWare account. Everyone interested in playing a sport during the 2017-18 school year will need to sign up for a SportsWare account, as this is the first year we will be utilizing the online option. Please see the instructions at the bottom of this letter to effectively sign up for your new SportsWare account, using a valid and current email address.

 

WITHIN THE SPORTSWARE ACCOUNT, THERE ARE 4 FORMS THAT ARE REQUIRED:

 

  • Required PIAA CIPPE form – this is your standard physical packet and is in two parts:
  • PART 1 – PIAA CIPPE 2017-18 Sections 1-4. This part INCLUDING signatures can be filled out electronically. You need to type your name as your signature. Please make sure you have all sections filled out completely. THIS SHOULD NOT BE PRINTED. Please make sure you click SAVE AND SUBMIT and follow instructions. If you are missing information on the form, you will not be able to submit the form and it will appear as INCOMPLETE on your home page.
  • PART 2 – PIAA CIPPE 2017-18 Sections 5 & 6. You are REQUIRED to bring this part to the physical. THIS NEEDS TO BE PRINTED AFTER YOU HAVE FILLED OUT NECESSARY ITEMS. Please make sure you click SAVE AND SUBMIT and follow instructions. Please bring this to your athletic trainer as soon as possible so it can be processed and checked for any errors.
  • Consent for ImPACT Testing & Release of Information 2017-18. It is required that this is read and understood. When finished reviewing, please click SAVE AND SUBMIT and follow instructions. This is a once-a-year form. Any questions, please ask.
  • Consent for Return to Play & Requirements 2017-18. It is required that this is read and understood. When finished reviewing, please click SAVE AND SUBMIT and follow instructions. This is a once-a-year form. Any questions, please ask.

 

**NOTE: These documents are located under the “FORMS” tab on the home page of your SportsWare account.**

 

Athletes who are unable to attend the school sports physicals screening held at OSS Powder Mill or desire to schedule their physical examination with their family physician must be aware that they do so at their own expense. All SportsWare/paperwork requirements outlined above still apply. Please make sure that you are bringing the PIAA CIPPE form Sections 5 & 6 that are found within your own SportsWare account. This is the most up to date form. PIAA requires this specific paperwork for interscholastic participation. Therefore, any other forms will not and cannot be accepted per PIAA rule! Also, earlier versions of the PIAA physical form cannot be accepted due to changes in the paperwork requirements.

 

IMPACT TESTING REQUIREMENTS

Dallastown Area School District requires that all students in 7th, 9th, and 11th grades, as well as any new student-athletes this year, take the ImPACT Baseline Exam. This year, the completion of this exam is required prior to the start of your sports season. We will be setting up labs at the high school over the summer to conduct these tests in a group setting. These dates will be determined later and will be communicated via the Dallastown Athletics Website. If you are unsure about the need for a baseline test, please ask the athletic trainers.

Prior to participating on a team from Dallastown High School, athletes must provide the Athletic Training Department with current address, emergency contact, medical alerts and health history information. To expedite this process Dallastown High School uses an online data entry system.

To enter your information, visit www.swol123.net. The first time you visit the website you will need to enter your parent’s email address and click Get Password.

 

** ALL INFORMATION SHOULD BE COMPLETED FOR THE UPCOMING SEASON.

 

For example: If you are a current 8th grader anticipating signing up for Junior High football (9th Grade), you should fill out the completed paperwork as a 9th grader.

No Username

Enter your School ID: dtwildcats

You should have received a School ID from the athletic trainer. This is required to join the correct school.

Join

Enter your First Name, Last Name, Email address and click the Send button.

Input Information

Your request to join SportsWare will then be sent to the Athletic Trainers for review.

Saved Information

Once your request is accepted you will receive an e-mail with the Subject “SportsWare request accepted”.

Open the e-mail and click the www.swol123.net link to continue to SportsWareOnLine.

by updating it with the Athlete’s email address
(defaults with Parents initially). Then Update

ADDITIONAL INFORMATION:

    • Each student is required to have his/her own SportsWare account, even within the same family. Each account is specific to the individual.
    • It is ideal for the parent to be the administrator of their child’s SportsWare account. We utilize SportsWare to convey important information via email throughout the year.
    • The same email address can be used for each student’s account; however, it is required that you use different passwords.
    • When first creating your account, you will be asked to enter the School ID (dtwildcats), then input first and last name, email address, and group. Once you complete this, click SEND. This is as far as you can go until the athletic trainers accept your request. You will receive an email notification when you have been accepted. At this time, you may log in and complete your profile.
    • If you receive an incomplete message when logging out of SportsWare, please log back in to correct the missing information. Your child cannot be marked as cleared until all information is completed. This information is used by coaching staff and athletic trainers as emergency contact information.
    • Please update SportsWare information as it changes. It is parental responsibility to complete this.
    • Please check junk mail for SportsWare acceptance emails. Depending on personal filters, this may go to your junk mail folder.
    • Due dates for the Recertification form for Winter and Spring seasons will be communicated via the email address used to log in to your SportsWare account. Again, please make sure your log in is a valid email address.
