ALL SPORTS PHYSICAL EXAM INFORMATION
HIGH SCHOOL/MIDDLE SCHOOL
2013 – 2014 SCHOOL YEAR
The Pennsylvania Interscholastic Athletic Association rules state that all students must have a new physical exam every year in order to be eligible to play a sport. Students who plan to play any sport or do cheerleading/dance team during the 2013 – 2014 school year must have their physical exam done on or after June 1, 2013.
Physical exam forms are available in the Athletic Training Room, High School Main Office, Nurse’s Office, and DAHS athletic website for any student who plans to participate in any of the sports seasons for next school year. A completed form must be turned in to the Athletic Trainer before the student is eligible to begin practice. Practice begins on August 12, 2013 for fall sports. Physical forms for all sports participants should be turned in no later than August 5, 2013.
Physicals will be done at Orthopedic Spine and Specialists (OSS), 717-848-4800, and only offered on the times listed below. All students who plan to try out for any sport, cheerleading, or dance team during the 2013-2014 school year will have an opportunity to have their physical exam done at OSS located at 1855 Powder Mill Road, for a fee of $15.00. Payment must be received at time of the physical and we will only accept cash or checks. Checks should be made out to DAHS. Credit Cards will not be accepted. Students with a last name A – L should meet at OSS on Saturday. June 1, 2013 at 2:00-4:00 p.m. Students with a last name M – Z should meet at OSS on Monday June 3,2013 at 6:00 p.m. Students should have their physical exam paper with them and already have sections 1-5 filled out. You can access the sports physical forms from the DAHS athletic website. Students with scheduling conflicts should use the other date. All students should turn in their completed physical form before they leave the building.
Please note that all fall, winter, and spring sports participants should have their physical done at this time. Winter and spring sports participants will then only need to be re-certified by a physician for three reasons. 1. The athlete has sustained a significant injury that has kept them out of practice for a week or more, 2. The form issigned by anyone else other than an MD or DO, 3. The athlete does not use thePIAA Comprehensive Form. Re-certification involves a parent signature to verify that the student has had no serious injuries or illnesses since the original sports physical (done on or after June 1, 2013).
Please be aware that even if a student has a physical done at O.S.S., he/she may be required by the school physician to obtain clearance from the family doctor or specialist before he/she is made eligible to begin practice if there is an existing health problem or recent injury.
